Please note:

• Please bring a list of current medications along with your current glasses.
• New patients will be asked to complete an information and medical history form. Click here for forms.
• A complete eye examination takes up to two hours.
• Dilating your pupils with drops allows for comprehensive evaluation. The dilating drops may temporarily blur reading vision and cause sensitivity to bright light. You may wish to arrange for transportation assistance.
